There's a dirty secret in Hollywood--Nielsen's numbers are wildly, I mean WILDLY, inaccurate. No political pollster would ever take Nielsen's polling samples seriously.
And now with Tivo and other smart tv devices, the wheels are really off the wagon. Nielsen doesn't count viewers who "time shift" the show (record it and watch it at a later date during the same week.) They assume that you're fast-forwarding through the commercials and therefore of no statistical value to the advertisers (and so the networks.
Just one example. College students are a coveted demo for advertisers but Nielsen doesn't factor in that college students tend to watch tv in groups--in their dorm rooms, common rooms, frat houses and other campus hangouts.

So why doesn't anyone do anything about it? Because the advertisers are afraid that the networks are going to find out that a lot more people are watching the shows than they thought, and the networks are terrified that the advertisers are going to find out that no one was watching the commercials in the first place. So they shake hands and agree on this system."
